Lean mass is not the same as muscle
The exploratory STEP 1 body-composition substudy reported reduced fat mass and a 9.7% decrease in total lean mass from baseline with semaglutide, while lean mass increased as a proportion of total body weight. Lean mass includes tissues other than skeletal muscle. This small exploratory substudy cannot predict how many pounds of muscle an individual will lose.
Individualize nutrition goals
The joint professional nutrition advisory discusses protein intake, resistance exercise and support for reduced appetite. It describes proposed protein targets during weight reduction while noting uncertainty about which body-weight reference to use. A fixed grams-per-kilogram prescription is not appropriate for everyone.
A clinician or registered dietitian can consider your usual intake, preferences, health conditions, kidney function and activity when helping you choose a target. Protein intake alone does not establish muscle preservation; resistance exercise and overall nutrition matter too.
Address practical barriers
If low appetite or gastrointestinal symptoms make eating difficult, discuss manageable meal patterns and tolerable foods with your care team. Eggs, dairy, legumes, fish and other foods can contribute protein; choices should fit dietary needs and individual tolerance. Do not assume a supplement is necessary solely because you take a GLP-1 medication.
Discuss persistent poor intake, unintentional weakness or suspected deficiencies. Supplement decisions should consider diet, risk factors and clinical assessment, rather than a universal multivitamin rule or an automatic laboratory panel.
